Pilot application
Applications are reviewed individually. Submitting does not approve a partnership, activate a contract, book a meeting or authorize a data connection. Do not include patient information, case details or other confidential records.
If an amount is agreed after review, an administrator can email a secure invoice payable by card or bank-account ACH debit. No payment is due just for applying.
